چکیده

Abstract BACKGROUND Cerebellopontine angle (CPA) surgery for extra-axial lesions in elderly patients could be hampered by concerns regarding functional outcomes. However, an overall ageing society warrants improved approach. Intraoperative neuromonitoring is crucial facial nerve (FN) preservation: intraoperative cortico-bulbar motor evoked potentials (FMEPs) can effectively coupled to standard techniques. In a previous work, we validated the use of FMEPs as preditors early and late post-operative FN function (FNF). Aim this study was re-evaluate FMEP role beyond 65 years age. METHODS An institutional series 83 reported. A pair needle electrodes used record from orbicularis oculi (OOc) oris (OOr) muscles; baseline, final minimum values were recorded. From amplitudes, minimum-to-baseline (MBR) final-to-baseline (FBR) amplitude ratios, recovery value (RV=FBR-MBR), calculated. These indices correlated (at discharge) 1-year) FNF. RESULTS 23 (27.7%) older than years. Among these, 52.2% 81% had good FNF, respectively. Early FNF not associated age (p > 0.99 p=0.72, respectively). patients, higher FBR (from OOc OOr) MBR OOc) (p=0.01, p=0.009, p=0.03, Conversely, OOr only parameter significantly with (p=0.049). ROC analysis confirmed that, most accurate index predicting measured (AUC=0.869, cut-off 12.5%). The (AUC=0.865). CONCLUSION Cortico-bulbar are reliable predictors allow safe feasible important cohort.

Transcranial electrocortical stimulation to monitor the facial nerve motor function during cerebellopontine angle surgery.

OBJECTIVE This study was conducted to investigate the success rate of using the facial motor evoked potential (FMEP) of orbicularis oculi and oris muscles for facial nerve function monitoring with use of a stepwise protocol, and its usefulness in predicting facial nerve outcome during cerebellopontine angle (CPA) surgeries. METHODS FMEPs were recorded intraoperatively from 60 patients undergo...

Intraoperative changes in transcranial motor evoked potentials and somatosensory evoked potentials predicting outcome in children with intramedullary spinal cord tumors.

OBJECT Intraoperative dorsal column mapping, transcranial motor evoked potentials (TcMEPs), and somatosensory evoked potentials (SSEPs) have been used in adults to assist with the resection of intramedullary spinal cord tumors (IMSCTs) and to predict postoperative motor deficits. The authors sought to determine whether changes in MEP and SSEP waveforms would similarly predict postoperative moto...

Facial nerve schwannoma arising from the cerebellopontine angle.

A 74-year-old female suffered hearing disturbance in the right ear persisting for several years, followed by sudden onset of right facial nerve palsy. Her symptoms gradually worsened and neuroradiological imaging revealed a 4-cm cerebellopontine angle (CPA) tumor. Intraoperatively, the tumor was found to originate from the facial nerve. Total removal of the tumor was achieved, followed by a spl...

Vestibular evoked myogenic potentials in patients with rheumatoid arthritis

Background: Rheumatoid arthritis (RA) is an autoimmune systemic disease. Most common autoimmune diseases are multisystem disorders that may also present with otological manifestations, and autoimmune inner ear disease accompanied by vestibular dysfunction. This study aimed to compare the vestibular function between RA patients and normal subjects using cervical vestibular evoked myogenic...
